Collection Instructions
Consult Pharmacy before ordering any Gentamicin levels.
- Physician orders for Gentamicin are sent to the Pharmacy who will then determine which dosing program to follow.

Pharmacy enters orders into system for program and/or levels needed and phones the Laboratory to set up drawing times.

Nursing notifies Laboratory of all dose start and stop times.
