Interpretive Data
Troponin-T concentration is determined using the Roche 5th Generation Troponin-T assay. The 99th percentile for Troponin-T for this test is 14 ng/L for females and 22 ng/L for males.
According to the fourth universal definition of myocardial infarction, the term "acute myocardial infarction" should be used when there is acute myocardial injury with clinical evidence of acute myocardial ischemia and with detection of a rise and/or fall of cardiac troponin values with at least one value above the 99th percentile and at least one of the following:
- Symptoms of myocardial ischemia
- New ischemic ECG changes
- Development of pathological Q waves
- Imaging evidence of new loss of viable myocardium or new regional wall motion abnormality in a pattern consistent with an ischemic etiology
- Identification of a coronary thrombus by angiography or autopsy (not for type 2 or 3 MI's)
Serial measurement of troponin and the change in troponin concentration over time (delta) is crucial for the diagnosis of acute myocardial infarction.
