Collection Instructions
Consult Pharmacy before ordering any Vancomycin levels.
1. Physician orders for Vancomycin are sent to the Pharmacy who will then determine which dosing program to follow.
2. Pharmacy enters orders into system for program and/or levels needed and phones the Laboratory to set up drawing times.
3. Nursing notifies laboratory of all dose start and stop times.
