Consult Pharmacy before ordering any Vancomycin levels.
- Physician orders for Vancomycin are sent to the Pharmacy who will then determine which dosing program to follow.
- Pharmacy enters orders into system for program and/or levels needed and phones the Laboratory to set up drawing times.
